Role of Endoscope in Management of Chronic Otitis Media in Rural Area of Bangladesh

 Introduction:
Endoscope has great role in preoperative diagnosis & documentations of Chronic Otitis Media (COM)  to create awareness and to motivate the people for ear surgery. Endoscopic Tympanoplasty can be done as day care procedure. Postoperative follow up by endoscope to show healed ear dram brings satisfaction of patients & surgeons

Methods:

Prospective study from March2013 to June 2018 done for primary care in rural area of Naogaon district Of Bangladesh . Endoscopic examination of ears & audiological test was done for diagnostic & therapeutic purpose in ear camp in villages to select patients with COM. Among them Tubutympanic variety were treated conservatively 1st then tympanoplasty were done in 260 cases at Peerless Hospital & research Institute.

Discussion:

Endoscope is better equipment for counselling the patients in management of Chronic Otitis Media

Results:

Awareness for ear care was significantly improved (p value <0.001) Motivation for ear surgery was also highly significant (p value <0.001)   Learning curve was higher. Graft survival and Hearing improvement were not significant (>0.05) Complications, residual & recurrence rate also not significant (P >0.05)

Conclusion:

Endoscope is Less costly portable tool to create awareness & motivation among rural people. Easily motivated to take care of ears & operative treatment where graft survival rate & hearing gain is more. So Endoscope may be used alternatively for proper management of  COM & hearing impairment at least in rural area of 3rd world Country like Bangladesh.
